Secure ID#'s


The client will be given an ID# by his/her therapist during the initial confirmation of their mutual date/time.  After which time the client must present their ID# to their therapist BEFORE they proceed into therapy during EACH session.


Couples/Married will receive only one ID#


Families will receive only one ID#

Services for ages 2yrs to 65yrs

HIPAA, a federal law that prohibits health care professionals from disclosing a patients medical records.


Federal Permissions to violate HIPAA:

1. a person is in imminent danger to him/her self

2. a person is an imminent danger to someone else.

3. the records have been subpoenaed by the courts.

4. a patient has abused a minor, senior or disabled person.


What happens to files once counseling has ended:


Adult Files are kept for 7 yrs


Child and Adolescent Files are kept for 7yrs past the age of 21.


After 6 months all files will be converted into an electronic only format.


After files have been purged, the client's name, id#, length of counseling, and initial reason for attending counseling will be kept electronically for an additional 5 years past the purge date.
